ORAL NUTRITIONAL SUPPLEMENT (ONS) DIARY
If your healthcare professional (e.g. GP, dietitian or nurse) thinks that you are malnourished or at risk of malnutrition and muscle loss, you may have been recommended an ONS (sometimes known as a sip-feed) to help you get the nutrients you need.
YOUR HEALTHCARE PROFESSIONAL WILL TELL YOU HOW MANY SUPPLEMENTS YOU NEED AND HOW OFTEN THEY SHOULD BE TAKEN, AS THIS WILL LIKELY BE VERY SPECIFIC TO YOU AND YOUR HEALTHCARE NEEDS.
It is Really important to follow the guidance given to you by your healthcare professional.
That’s why we have developed an ONS diary to help you keep track of each time you take your supplement on each day of the week. You can also record your weight and any changes you notice to see your progress and discuss this with your healthcare professional during appointments.
You can download the diary below. Print it off and keep it somewhere safe and convenient so you can mark off how much of your ONS you have taken each day (or twice a day, depending on your recommendation). Don’t worry if you only take a small amount of your recommended dose; just mark up what you have taken in your diary.
You can also take the diary to your next appointment to discuss your progress with your GP, dietitian or nurse and/or any challenges you might have had in keeping to your recommended dosage.
