Our symptom tracker is here to help you keep track of your child’s gastrointestinal symptoms in the weeks or days leading up to their next appointment with their healthcare professional (e.g., GP, consultant, dietitian, etc.). It is quick and easy to use.
The symptom tracker can be useful for helping you track your child’s symptoms, such as whether they have experienced any diarrhoea, sickness, tummy pain etc. so that you can discuss these with their healthcare professional at their next appointment.

UNDERSTANDING THE SYMPTOM TRACKER
This downloadable symptom tracker provides a four-week chart to log your child’s symptoms, along with changes in their weight and height. Daily symptom tracking is made easy with a simple scoring system:
1. Symptoms improved
2. Symptoms stayed the same
3. Symptoms worsened
Simply download, track, and take it to your child’s appointment to discuss changes with your healthcare professional.
At the end of the symptom tracker, you’ll also find a section to make any additional notes about your child’s progress or add any questions you might have for your child’s healthcare professional. Going into your child’s healthcare appointment with as much information as possible can help the healthcare professional provide the right advice and/or treatment for your child.
BRISTOL STOOL CHART
Sometimes, your child’s healthcare professional will ask about your child’s bowel habits because it can help them understand if your child may have any digestive problems.1
By using the chart below, known as the Bristol Stool Chart, you can note any changes to your child’s poo. This will enable you to have an open and accurate conversation with your child’s HCP without having to bring in a sample.
The normal range is typically type 3 ‘corn on the cob’ or type 4 ‘sausage’.1
