KEY MESSAGES

 

1. Guidelines recommendation of nutritional intervention for sarcopenia: 

  • Exercise with nutritional interventions (high-protein diet, HMB supplements, etc.) are generally recommended by multiple guidelines for patients with sarcopenia.17-23 
  • In BSGG 2021 guidelines, HMB supplementation is recommended to increase muscle mass based on high-quality evidence.23

 

2. AWGS 2022 consensus statement:24,*

  • To maintain sufficient protein intake, AWGS recommended a daily protein intake of ≥1.0 g/kg body weight for healthy older adults and ≥1.2 g/kg body weight for those with sarcopenia and/or frailty.* 
  • For older adults who are candidates for supplementation, high‐quality protein, amino acids such as leucine and l‐carnitine, or oral nutritional supplements containing HMB may be considered and should be taken according to the specific prescribing information. 
  • Oral vitamin D supplementation (800–1000 IU/day) may be beneficial for older adults with vitamin D insufficiency. Higher doses may be required for those who are deficient in 25‐OH vitamin D.

 

3. ESPEN 2023 guideline recommendations:25

ONS with HMB nutritional intervention is recommended for malnourished polymorbid medical inpatients.25

  • For malnourished polymorbid medical inpatients or those at high risk of malnutrition, high protein nutrient-specific ONS containing HMB should be administered when they may help maintain functional status and muscle mass, reduce mortality and improve QoL. 
  • Patients receiving specialized ONS containing HMB showed in a greater extent improvement in handgrip strength, nutritional and performance status, compared to the control receiving the placebo ONS
